I think he just has issues keeping a tight seal from the corners of his mouth to his eye-teeth. Maybe a medical condition such as a stroke.

I happened to see this thread, and followed the link. The trumpeter is Colin Graham, and in that picture he is playing with the Yiddish Twist Orchestra.
I reached out to them on Facebook, and they said that it was some kind of insulator for playing cold gigs.
Before I found this out, I thought maybe he used a special embouchure setup that he wanted to keep secret. (I read somewhere that there was a jazz pianist who used to play with a handkerchief covering his left hand so that people couldn't "steal his chords"!)
