Yessir. Routines are sometimes not the right answer for players for this reason. "Daily Maintenance" is not how I approach practice (most of the time).

Yes, including when doing the most 'easy' playing. The goal should be to do it as good as possible.

Even with a regular maintenance (or warm-up) routine, the most benefit will come from striving to do it well, not to just 'check it off the list'.

Through the 'striving' you are practicing TWO important aspects of playing - the physical and the mental. Do more that just 'pump iron'.

KNOW (see) what the next note is BEFORE you have to play it.
PLAY the next note 'on time' and 'in rhythm'.
Oh ya, watch the conductor - they set what is 'on time'.
