Chocolate Shake Freddie Hubbard Version

Hi, I’m doing a transcription of Chocolate Shake (Duke Ellington), but the Freddie Hubbard version. Would anybody have a lead sheet? I can’t find one.

Again, I know this tune is obscure, I was hoping someone had a lead sheet or changes.

Hi trying again, what I need are the changes. The recording I have is hard to hear and there’s some out of tune instruments in the intro and other hard to hear things.
